Successful outcomes include moving to a next stage that reflects a young person’s strengths, aspirations and support needs. This may involve further education, specialist college, a supported internship, employment, higher education or another suitable learning or support pathway.

Transition planning is personalised. We consider achievements, communication and independence alongside the support needed to make the move successful, working with families and receiving providers.
